A physical exam will be done to assess your pet’s overall health prior to giving any anesthetic medications. To better assess your pet’s internal organ health, we offer a pre-anesthetic blood panel to be performed prior to the procedure. It is important to understand that the pre-anesthetic profile does not guarantee the absence of complications. It may, however, greatly reduce the risk of complications and identify medical conditions that could require treatment in the future. For patients under 7 years of age, this is optional but strongly recommended for patients. In patients 7 years and older, we require pre-anesthetic bloodwork within 3 months of the procedure unless otherwise determined and discussed by the veterinarian.

    Please choose a surgery package below. Vaccines and heartworm/tick disease (dogs) test/ Feline leukemia/FIV test (cats) will only be updated if due. The pet will also only be microchipped if not previously microchipped.

I understand that there are potential risks with anesthesia and/or performing surgery on an animal including death. I assume full responsibility for these decisions. I understand that payment is due at the time of service. *

    In the case of an emergency, I would like Cottonwood Vet Clinic to initiate life saving measures such as CPR. I understand that I am responsible for this additional cost, regardless of outcome. *
